The holder cannot record it themselves. You performed the checks, so you are the only party that knows the answer — and the chain enforces that by accepting this recording only from an approved participant. This page holds no key and signs nothing: it checks the request against the chain and prints the command your own group signs.
Read from the chain. Only an institution on this list may record where an account belongs, and the name in the request has to match one of these exactly.
Asking the chain…
These are the refusals this page applies before the chain does. The chain applies them too — but it answers with a code, after a signature, and without the sentence saying which one was broken.
One more thing is a warning rather than a refusal. A request that names no institution at all is still recordable, because the chain does not require the document to name you. It is worth reading twice: check the request is yours to act on before you sign.
